Welcome to the Ultrasound Pelvic Index (UPI)

What is UPI? --- UPI is a system that combines data from pelvic floor questionnaires and ultrasound measurements to determine the type and severity of pelvic floor dysfunction a patient may have. The UPI system provides a standardized scale to identify and classify pelvic floor dysfunction. This helps create a common language for medical professionals to discuss a patient's condition and treatment options.


The Ultrasound Pelvic Index (UPI) is like the hurricane strength scale, where higher numbers signal more severe conditions:

  • UPI 1 (Score < 10) = Good pelvic floor support with minimal symptoms
  • UPI 2 (Score 11-30) = Mild support, Patient may complain about frequency, urgency, and/or urge incontinence
  • UPI 3 (Score 31-60) = Moderate support problems, with stress urinary incontinence and/or fecal incontinence
  • UPI 4 (Score 61-110) = Pelvic floor dysfunction, severe support problems with stress urinary incontinence and/or fecal incontinence

UPI Levels and Meaning

The basic index level measures pelvic health through ultrasound and provides a starting point to understand pelvic muscle strength.
